Referral Pathways.
At WellMinds, we offer both face-to-face, telehealth and video call services for our clients.
Clients can initiate the referral process themselves by leaving a message with our administration team at admin@wellminds.com.au or by filling out the form below.
Clients seeking the services of Mental Health Social Workers, Clinical Social Workers or Psychologists can receive a rebate of the full fee through a Mental Health Care Plan (item number 2751) referred by a GP/Doctor.
The plan allows up to 10 visits per MHCP and clients can receive a rebate of $80-$112 off the full fee.
Our fee structure ranges from $130 to $180 dollars.
Clients seeking referrals to Credentialed Mental Health Nurses must do so through a Management Plan/ECP referred by a GP.
They can receive 5 individual services per calendar year using items 10950-10970.
All referrals can be sent to admin@wellminds.com.au and clients can request a specific clinician through their GP referral.
